EMR Recertification Application Confirmation

Your S-SV EMS EMR Recertification application has been completed. The following additional requirements must be met before your EMR Certification can be issued:

  • Submission of the following supporting documents (documents not uploaded with your online application may be emailed to info@ssvems.com).
    • Copy of your current U.S. state-issued Driver’s License or photo ID.
    • Copy of your current or expired EMR Certification Card.
    • A completed CE Documentation Form or copies of individual CE certificates.
    • A completed EMR Skills Verification Form.
  • Payment of the EMS Recertification Fee.
    • Click the ‘Pay Fee Online’ button below to pay the $39.00 fee with a credit/debit card ($35.00 recertification fee + $4.00 online payment processing fee), or mail/deliver a $35.00 Money Order to: S-SV EMS, 535 Menlo Drive, STE A, Rocklin, CA 95765.
